The blank looks simple. The track you are on is not. EASA Form 1 is an Authorised Release Certificate (FAQ 19466). It declares airworthiness of a product, part or appliance. It is not a delivery note, not a release of an aircraft, and not by itself authority to install on a particular aircraft, engine or propeller. The form says that last point in the user/installer block.
Who may complete it depends on production vs maintenance. Same form family. Different sides, different Block 11 words.

Which track are you on, production or maintenance?
Production (POA holder): Form 1-21, Regulation (EU) No 748/2012 Appendix I to Annex I (Part-21) (legislation.gov.uk Appendix I). Maintenance (Part-145 / CAO / Part-M): Form 1-MF/CAO/145, Appendix II to Annex I (Part-M) of Regulation (EU) No 1321/2014.
Do not mix production-released and maintenance-released items on one certificate. Do not mix approved-data and non-approved-data items on one certificate. Correlation must exist between the certificate and the item.

FAQ 19044 (28 Jan 2021): after maintenance, those four Block 11 entries are the permissible set. “Inspected/Tested” means inspected and/or tested as in Part-M/Part-145. Block 12 should contain the detailed information on the status/work in Block 11.
Do not copy FAA Part 21 terms (NEW / PROTOTYPE / USED) onto Form 1. That split lives on 8130-3 Block 11.

Fourteen blocks, one track. Blocks 1–12 identify the organisation and the item. Blocks 13 and 14 are the certification sides. You complete one side. The other is shaded. Prefer ICA / CMM / IPC terms in Block 7. Put the part number as on the item in Block 8.

What belongs in Block 12 remarks?
FAQ 19044: Block 12 details the Block 11 work. QCM’s restatement of the appendix: “In accordance with the CMM” is not acceptable. Examples it lists (vendor restatement, not a substitute for GM): AD/SB compliance, repairs, modifications, replacement parts, LLP status, shelf life, NDT methods, deviations, foreign CAA statements, batch numbers. What does a filled maintenance line look like?
A filled line is Blocks 7–12 plus the maintenance certification in 14, with 13 shaded. The values below are invented for teaching. They are not a real release. Do not photocopy them into service.

How do you correct a Form 1, including an electronic one?
Identify errors in writing to the originator. A correction needs a new tracking number, signature and date. Block 12 on the correction states which blocks of the original it corrects and that it does not cover conformity/condition/release to service. Keep both certificates.
Electronic Form 1: do not allow signing a blank form; preserve integrity after signature. QCM restates printed copies watermarked “PRINTED FROM ELECTRONIC FILE.” Data exchange may use ATA Spec 2000 Chapter 16.
FAQ 66700 is which foreign ARCs a Part-145 organisation may accept, not how to fill Form 1. Equivalents: “Form 1 or equivalent”.
